Below are several documents to assist you on your credentialing journey
PLEASE NOTE:
You must upload ONE PowerPoint file containing all case submissions using the Fellow Case Template. Before uploading, rename the completed PowerPoint file using your assigned candidate number.
Additionally, you must upload a separate Patient Release Form for each patient.
